A caregiver was overheard making threats of corporal punishment to a child after the child swatted at the caregiver.

During the walk-through, one caregiver was not aware of the number of the children responsible for. The caregiver stated that there were 10 children present in the class but there were 11 children present. This was confirmed after the inspector counted the children with the caregiver present in the group and staff reported a child had been in the class for 10 minutes and was not counted. Additionally, another caregiver in another class, reported having 13 children in the group but there were 14 children present. The children in the group were re-counted with by the caregiver with the inspector and there were indeed 14 children in the group/class. One staff was not aware of the ages of the youngest child responsible for.

Based on video evidence a caregiver grabbed multiple children and pulled them in a rough manner when not following direction from the caregiver. Additionally, after grabbing and pulling them they were then pushed to the ground or slammed down on their bottom.

Based on video evidence a caregiver took a toy doll from a child and hit the child with the doll as a means to control a behavior. Additionally, the caregiver pushed their walker into a child and then pushed a cart full of nap equipment into another child.

Based on evidence obtained a caregiver did not discipline children at naptime appropriately, the director stated that she was aware that naptime was the hardest time for the caregiver, yet did not supervise the employee appropriately, thus leading to the inappropriate use of discipline to children during naptime.

Based on video evidence a caregiver did not use good judgement or self-control when becoming frustrated and pushing a horseshoe shaped table towards a child to trap the child in the semicircle of the table, as means of correcting a behavior. Additionally, on two occasions she took a nap mat from a child and threw it across room.

Evidence obtained during the investigation shows that staff have smoked on the operations property.

A caregiver failed to intervene when a group of children were using equipment inappropriately, which resulted in a child falling and breaking their arm.

An infant was observed walking around the classroom drinking from their bottle.

The condition related to caregivers being given a pretest before the review begins and a posttest demonstrating knowledge of minimum standards after a member of the governing body reviews the discipline and guidance subchapter and the operations policies regarding discipline and guidance was not in compliance as not all caregivers had documentation of the pretest and posttest.

There were several children not signed out on the tracking system. The staff is initialing it with "st" they need to write the staff persons initials.

A child was left unattended for approximately. 7 minutes.

Based on evidence obtained during the investigation, a child was left on the playground by themselves for less than a minute. Due to this, the caregiver, nor any other operation staff, had auditory or visual awareness of the child's ongoing activities. This prevented the caregiver from being able to intervene had it been necessary.

Based on evidence obtained during the investigation, a caregiver pushed a child's head back, roughly, multiple times as a form of discipline.

This standard was found to be deficient as part of a DFPS investigation.

Based on evidence obtained during an investigation, a caregiver spanked a child multiple times for discipline.

Based on evidence obtained during the investigation, a caregiver grabbed a child and pushed and pulled the child while disciplining them.

Based on evidence obtained during this investigation, staff did not report suspected abuse, neglect, or exploitation to DFPS.

Based on evidence obtained during the investigation, a caregiver did not use good judgement or self-control when she swung a child around causing the child to collide with another child, out of frustration.

Based on evidence obtained during the investigation, multiple caregivers have yelled at children while they were in care at the operation.

Based on evidence obtained during the investigation, children under two years of age were placed in restrictive devices for time out.

Based on the evidence obtained during the investigation, children's diapers are not being changed promptly.

Based on the evidence obtained during the investigation, a caregiver did not demonstrate good judgment and self-control in the presence of children when the caregiver picked up a child up by one arm causing the child's elbow to become dislocated.

During the inspection a 6 month old infant was observed sleeping in a swing and was not removed from the swing and placed in a crib promptly. Staff were observed having multiple opportunities to relocate the infant to a crib between their caregiver duties but failed to do so.

There are multiple pieces of flooring in the 2's 1 classroom and in the 4's classroom that are sticking up and pose a tripping hazard.

Evidence obtained during the investigation supports the allegation that a caregiver did not use good judgment when they carried an infant to an area where a hot crock-pot was present causing the infant to be burned.

A caregiver was observed changing dipaers/pull ups and did not change their gloves or wash their hands between changing each child's dipaer/pull up. Additionally, this caregiver was observed having children sit on the floor while changing their pull ups.

The last fire inspection conducted took place on 7/21/2020.
