The child safety alarm installed in each of the vehicles used for transportation is not disabling when the button is triggered in the back of the vehicle after it is checked. The alarm system requires maintenance.

Four staff records lacked the signature of the signature of the trainer. One also lacked the participants signature. All four lacked some or all of the topic boxes for orientation. All four sections of orientation was corrected at inspection.

Based on the evidence obtained during the investigation, a caregiver used a harsh tone when yelling at children in care when attempting to get children to listen to instruction.

Based on the evidence obtained during the investigation, a caregiver grabbed a child off a shelf by the arm and onto the ground as the manner in which to remove the child from a low-level shelf in the classroom.

Based on the evidence obtained during the investigation, a caregiver added another child to a classroom and placing it over ratio.

Based on the evidence obtained, there is substantial evidence that a caregiver was handling children in care in a rough manner and grabbing them by the arm for discipline.

A frame for a canopy on the toddler playground has two broken pipe pieces that are suspended and only attached to one end. One piece is suspended at a 45 degree angle pointing to the ground. The canopy frame requires immediate repair to prevent the broken pieces from coming completely detached and falling into the outdoor space. The piece that was at a 45 degree angle was repaired at inspection and the other piece will require a taller ladder to reach for repair.

Water was not observed being served after outdoor activities or during snack time.

A caregiver did not timely intervene on the playground when a child's safety was at risk. The plastic kitchen was pushed over onto the ground on top of a child and additional children were sitting on the child through the openings of the kitchen. Additional children sitting on top of the kitchen and pushing each other.

Two classrooms were observed making threats to the children in care that they won't have snack time if they don't behave. One placed the child in time out and withheld snack time while the rest of the classroom had snack. In another classroom, the caregiver told the child that they can either eat at the designated chair for snack or could go to time out. The child wanted snack but just wanted a different seat.

Two classrooms were observed at inspection with children out of control and the caregiver in charge unable to maintain control of the group. Children were observed climbing on shelves, immediately defying time out requests and running around the classroom.

The toddler classroom was observed with the children engaging in afternoon snack but they did not have their hands washed prior to snack time.

Two children were observed running out of their primary classroom into the kitchen of the operation. The primary caregiver stayed in the primary classroom with the first group of children but was unable to adequately supervise the two that left the room and were in the kitchen.

A caregiver was observed at inspection leaving a child on an above ground diaper changing surface in the classroom bathroom. The caregiver walked away and into the classroom to discipline other children in care.

A caregiver was observed at inspection communicating with a child that required re-direction and positive guidance. The caregiver told the child, "you want to listen or do you want to be a baby." This statement does not encourage positive statement to the child.

The waiting period between the children coming in from outside and being seated for lunch was over a 30 minute period with books. The staff did attempt to sing songs prior to the books but the children immediately lost interest without the option of a different activity.

Water was not offered or served after coming inside from being outside for active play. Additionally water was not served during lunch time.

Multiple children were observed being placed in time out for 5-15 minutes when the ages of the children were 2 and 3 years of age.

The operation has areas that require more immediate attention regarding the maintenance of the building and grounds. Both playgrounds have an abundance of fire ants that have bitten several children in the areas that are frequently played on. Both playgrounds have an abundance of sticker burrs that are sticking the children's clothing and shoes. Then the sticker burrs are being tracked back into the classrooms and sticking on to the carpet. The interior of classrooms have dirty tiles, dirty carpets, and a fan has large layer of dust over the fan blades and the fan cover. The window sill has dirt, dust and dead insects.

Children under the age of 2 are limited to equipment no higher than 32 inches as the highest designated platform but the children under the age of 2 were observed playing on equipment higher than five feet in height.

There were several trash cans without lids that were accessible to children that had items that could attract pests and have. Additionally, a cord to a radio is expended down a tall cabinet to the used outlet. The cord is not secured, and the radio could be pulled down easily. Additionally, there are several outlets in rooms used by children under the age of 5 that do not have outlet covers.

There were a few incidents that required first aid and first aid was not given. A child in care tripped and injured his mouth and nose that resulted in bleeding. The child was brought it to be evaluated but after returning back on the playground the child's nose began bleeding again and the caregiver used his shirt to wipe the blood off his nose and mouth and then had him continue playing without requesting the first aid kit to additionally review his injuries. Additionally a second child had a bloody nose that was smeared across his face that was not attended to. Additionally, a third child pinched their finger under the bottom of the door as it was opened when entering the building from the playground and first aid was not provided.

Diaper changing was observed at inspection and the caregiver did not wash hands in between any of the diaper changing changes or after handling cleaning supplies. The children additionally did not wash their hands.

Two classrooms were observed at inspection with children placing toys in their mouths and the toys were not placed aside. These same toys were then observed being placed in another child's mouth.

A caregiver was unable to maintain control of the group of children she was responsible for. Observations at inspection include multiple children climbing on toys, striking each other, climbing on shelves and multiple minor injuries were observed due to the behavior of the children towards other children.

The playground used by older children has large amounts of weeds emerging from the loose fill of the playground structure within the use zone.

Older infants were observed seated and buckled at the multi occupancy table for over 20 minutes prior to the start of a meal time. A younger infant was seated in a restrictive device on the floor awake for over 20 minutes prior to being removed for a diaper change and then re-seated in the restrictive device.

It was observed at inspection that a caregiver was unable to maintain control of the children she was responsible for. Children were climbing on shelves and tables. One child jumped off a shelf 16 times despite verbal requests to stop by the caregiver.

An infant was observed at inspection asleep in a restrictive device.

A caregiver was unable to maintain control of the children she was responsible for. Children were climbing on shelves and tables. Children additionally began pushing multiple shelves over as well as repeatedly hitting each other.

Based on the evidence obtained during the investigation, toys are not being put side for sanitizing after a child has placed the toy in their mouth.

The toddler classroom that had chipped shelving had it replaced and the broken tiles covered to prevent injury. The ants on the playground has been treated and eliminated the ants. Cigarettes' butts were still present on the playground. These were removed at inspection.

Various areas of the operation has areas that require attention and repair. The pre-k classroom has toy shelves with chipped protective covering on various areas. Two pieces of floor tile is exposed that is cracked and broken off. The larger playground has several cigarettes near the picnic tables that children use. The cigarettes were removed and disposed of at inspection. Both playgrounds have fire ants that are very active across various parts of the outdoor play space.
