The last children's product certification was completed on 9/20/2024 and it is required to be completed annually. **This was corrected at inspection when the provider updated at inspection.

The operation has not provided the Parent's Rights information to parents as required of the operational policies and did not include a policy with iinformation to parents about the promotion of physical activities. The updates required for the policy to pets/animals in the home has been updated and provided to parents.

A child's file review did not have any immunizations in file to review when requested.

Cleaning supplies and rash ointments were not inaccessible to children in care as they were within reach. Additionally, the backyard had a broken fence picket that needed to be fixed and had exposed nails from the fence picket that was missing.

The provider added smoke detectors the living room, two play rooms and three nap rooms but the kitchen is still needing a smoke detector where the children have meals and snack.

Children under two years of age were observed participating in screen time.

Multiple infants were observed having their diaper changed but did not have their hands washed after.

The operation does not have a current documentation of pet vaccination.

The operation does not have a statement of health from a vet.

With one caregiver you can only have 4 infants and two children over the and of 17 months.

The provider is allow the children under 5 years to play in the front yard without a fence, while the backyard is under construction.

All but one record was missing the Health Statement.

The provider had a baby jumper in the door way and this is a prohibited equipment.

The operation allowed an assistant to be present at the operation before recieving notice from the CBCU of the assistant's eligibility.

The provider does nto have a written emergency plan avaliable to review.

There was some "keep out of reach items" in the bathroom, there was vaccums plugged in and cord on floor in play area, There was a few cords hainging. Outdoor area had a tub of water under a leaky faucet and fencing is out but not up yet to block areas children will not play.

Bottles were nto labeled.

There is a fire extinguisher but it is not mounted.

The provider was caring for 3 unrelated children without a permit.
