Based on the evidence obtained during the investigation, staff were not aware of a child's on-going activity and did not have visual or auditory awareness of the child, as a child was in a classroom for approximately fourteen minutes by themselves before they were found by staff.

An individual that had a prior eligible background check was inactivated on 06/17/2026 and has continued to be present at the operation without an eligible background check at the operation. The individual is currently inactive and requires a new background check submission to be present at the operation. The individual left the operation during the inspection.

Children were left the classroom unattended and were unsupervised in the halls and office.

Based on the evidence obtained during the investigation, a caregiver would discipline by grabbing children by the arm in a rough manner.

Based on the evidence obtained during the investigation, an incident occurred where children were repeatedly climbing on shelves during nap time in the classroom and the caregiver failed to intervene. The caregiver sat on the ground instead of removing the children from the shelves to ensure their safety. The caregiver doesn?t attempt to redirect the children until after a child falls off the shelf and lands onto another child laying on their nap mat resulting in the other child crying.

Based on the evidence obtained during the investigation, a caregiver has grabbed multiple children by the arm in a rough manner when disciplining.

A child walks out of the classroom and down the three halls without supervision, while the caregiver was changing a diaper in the diapering pod. Staff was unable to see and unaware he left the room.
