Through investigation it was determined that a child was left unsupervised in a classroom.

Cleaning supplies such as all purpose cleaners were stored in a low cabinet without a child lock. This was corrected at inspection when the chemicals were moved out of reach of children.

For the 2022 calendar year, March, April, May, and June fire drills were not documented.

The food allergy plan for a child with doctor diagnosed food allergies did not include what foods the child is allergic to and possible symptoms if the child is exposed to said food.

The diaper changing pad in one infant classroom had two 1-inch holes on the side. This was corrected at inspection when a new diaper changing pad was added.

The walls in the indoor gym had damage to the bottom of the wall. Holes in the wall were about three feet by 6-8 inches.

Of the five random student files reviewed, one did not have immunization information.
